Integrated Care

Our nation’s healthcare system is plagued by many problems, but principal among them is the problem of fragmentation, particularly of the so-called physical from the so-called mental.

ACT Innovations

ACT relied on the imagination and practical knowledge of people working day-to-day to care for people within their communities.

Innovation Sites

The projected number of participants for the 11 sites included more than 440 practice members, caring for more than 120,000 patients. The variety of practices in ACT enriched the experience base and broadened potential relevance to all practice types in Colorado and beyond.
